MEDICATION MANAGEMENT
Our Medication Management Program offers peace of mind to patients and the primary care physician by creating a comprehensive, non-conflicting medication plan tailored to meet each patient's specific needs.
FALL PREVENTION PROGRAM
This program provides a full assessment, including environmental, medical and personal risk factors.
BRIDGE PROGRAM
This program provides short-term home services to the post-surgical patient who is home-bound during the transition from hospital to outpatient services.
HOSPITAL SITTER PROGRAM
You can't be there 24/7. So if a little extra attention is required, we can provide one-on-one care in most settings, ranging from 3-to-24 hour coverage.
