Bridge Program

Our Bridge Program is designed to provide high-quality professional home care services that promote independence. Our Bridge Program provides short-term home services to the post-surgical patient who is home-bound during the transition from hospital to outpatient services.
Our goal is to begin the rehabilitation process and provide needed support to the patient during this inpatient –to-outpatient transition. Bridge treatment is individualized, but the usual time frame is approximately two weeks.
Services offered:
- Nursing
- Speech Therapy
- Physical Therapy
- Home Health Aide
- Occupational Therapy
- Social Worker
Program benefits:
- Bridging the gap between hospitalization and outpatient follow-up
- Maintaining client safety in the home during this time by providing support for disease management and ADL
- Beginning prescribed therapy services to increase ability to safely leave the home for outpatient services
- Goal to maintain client within current physician chosen care network
- Reduces outpatient therapy time
