Helping Hand’s Fall Prevention Program provides a full assessment, including environmental, medical and personal risk factors. Our program features a Cognitive Assessment to determine each patient’s ability to learn and process information.

Facts about falls:

  • 60% of falls occur at home
  • 30% of people over age 65 will fall each year
  • Falls are the leading cause of fatal and non-fatal injuries in people age 65 and older in the United States

Falls Prevention Program Components include:

  • Coordination with the physician on skilled needs
  • Environmental safety assessment
  • Cognitive assessment
  • Plan of Care designed to meet each patient’s needs
  • Skilled nursing for medication management and disease teaching
  • Physical therapy for improvement of balance and mobility and use of adaptive devices
  • Occupational therapy for development of plan based on cognitive abilities and reduction of hazards when completing activities of daily living
  • Social worker for community resources
  • Home health aide for personal care assistance.
