Fall Prevention Program

Helping Hand’s Fall Prevention Program provides a full assessment, including environmental, medical and personal risk factors. Our program features a Cognitive Assessment to determine each patient’s ability to learn and process information.
Facts about falls:
- 60% of falls occur at home
- 30% of people over age 65 will fall each year
- Falls are the leading cause of fatal and non-fatal injuries in people age 65 and older in the United States
Falls Prevention Program Components include:
- Coordination with the physician on skilled needs
- Environmental safety assessment
- Cognitive assessment
- Plan of Care designed to meet each patient’s needs
- Skilled nursing for medication management and disease teaching
- Physical therapy for improvement of balance and mobility and use of adaptive devices
- Occupational therapy for development of plan based on cognitive abilities and reduction of hazards when completing activities of daily living
- Social worker for community resources
- Home health aide for personal care assistance.
